Examination and Planning Phase

Initially, we will conduct a thorough examination to obtain all information needed to formulate a treatment plan. We will discuss this personalized plan with you so informed decisions can be made. Acceptance of and following through with the comprehensive treatment plan will result in the most comfortable, durable and esthetic result. Our fee for suggested services will be presented and any potential insurance benefits will be determined.

Home Care and Prevention

Good dental health requires constant attention. We will recommend a personalized self care routine based on your specific needs. Periodic re-examination and cleaning of your teeth are also vital. Ideally, we become partners to minimize future dental problems and expenses.

Continuing Education

We regularly attend various programs, individually and as a staff, to keep up in this rapidly changing field. Improving our skills and awareness are of paramount importance to us, and this is reflected in the quality of the service we provide.

Emergency Care

We attempt to work emergency patients into our schedules for diagnosis and for relief of discomfort. During regular office hours, phone us at 929-6626. If the office is closed, our answering machine will record messages and direct you to call my home if necessary. We will arrange for coverage by another doctor when we are out of town for any reason.

Fees, Insurance, and Patient Responsibility

As a service to you, we will submit charges for services to your insurance company; however, we consider the patient to be responsible to us for the account.

We will work with you and your insurance company to assure that all benefits payable are realized. Please provide us with a claim form (with your personal information completed) at the initial visit. Whenever insurance benefits are assigned to us, the portion of the charge which is payable by the patient (such as deductible, co-payment, or excluded service) should be paid to us at the time of service.

For patients who have no insurance, we require that payment be made at the time of service. We accept cash, personal checks, Master Card and Visa as methods of payment.

Late Arrival, Cancellation

The reservation nature at our office assures you are given devoted time in our schedule. In case of a late arrival, services may be shortened with a full service fee. In case of a late cancellation or a no show, your account may be charged a portion of the proposed treatment based on an hourly fee. A credit card or cash retainer is required when booking services. We will continue to be flexible in cases of emergencies.

Office Hours and Appointments

Office hours are by appointment as follows:
MONDAY -THURSDAY, 8 A.M. to 5 P.M.
When you request an appointment, we reserve time for you in our schedule. If you are unable to keep an appointment, please notify us at least 48 hours in advance. This will allow us to schedule another patient who needs care.

Thank You

Thank you for the confidence shown in selecting our office for your care. We will do our best to treat you comfortably and well. The highest compliment you can pay us is to refer us to your friends.
